1. Data controller
Under Turkish Personal Data Protection Law no. 6698 ("KVKK"), your personal data is processed by Skaydent Sağlık Hizmetleri A.Ş. (Özel Sky Dent Ağız ve Diş Sağlığı Merkezi) (Kartaltepe Mah. Göktepe Sk. No:1 (on the E-5 highway), Küçükçekmece / Istanbul) acting as data controller, within the scope described below.
2. Personal data processed
Through the appointment and contact form, your name and surname, telephone number, e-mail address and the information you share in your message are processed. If you share information about your treatment needs in your message, that information constitutes health data, which is a special category of personal data, and is processed only with your explicit consent.
3. Purposes of processing
- Receiving and scheduling your appointment request and getting back to you
- Answering your questions and providing information
- Planning and delivering the healthcare service
- Fulfilling obligations arising from legislation
4. Legal basis
Your identity and contact data are processed on the legal bases set out in KVKK art. 5/2, namely that processing is "directly related to the conclusion or performance of a contract" and the "legitimate interest of the data controller"; your health data is processed on the basis of your explicit consent under KVKK art. 6/2.
5. Transfers
Form data is stored on our website service provider's server hosted in Türkiye so that it can be delivered to our clinic, and is accessed only by clinic staff. It may be shared with authorised public institutions where legally required. It is not transferred to third parties otherwise.
6. Retention period
Form data is retained for a maximum of 1 year from the conclusion of your request; records that lead to treatment are retained for the periods set out in healthcare legislation.
7. Your rights
Under KVKK art. 11 you have the right to learn whether your personal data is processed, to request information if it has been processed, to learn whether it is used in line with its purpose, to request correction if it has been processed incompletely or incorrectly, to request its erasure or destruction, and to claim compensation if you suffer damage.
